Exhibit E: Consent to Record Calls

This Call Recording & Transcription Consent (“Consent”) is made by [Client.Company]​ (“Customer”) in connection with Services provided by pie Health, LLC (“Supplier”) under the General Services Agreement (“Agreement”) between the parties.

This Consent is effective as of the effective date of the Agreement and applies to all communications during the term of the Agreement and any associated SOWs.

1. pie Health, LLC may record and/or transcribe telephone and video calls related to the Services described in the General Services Agreement.

2. Recordings and transcripts may be used by pie Health internally for documentation, compliance, training, and quality assurance purposes.

3. If any such calls involve the discussion of Protected Health Information (PHI), the recordings and transcripts will be treated in accordance with the Business Associate Agreement (Exhibit D) and all applicable privacy laws.

4. Consent to call recording and transcription is a condition of this Agreement. Revocation of this consent by Customer shall be deemed a material breach of the Agreement and may result in immediate termination of Services by pie Health.

5. Customer agrees that attending any scheduled call after execution of this Agreement constitutes reaffirmation of this consent.
